Nah, you can see the top surgery scars quite a lot through this entire arc. It can be easy enough to mistake them as just being the bottom of his pecs though, especially since we didn’t see Noah before he transitioned. It can be easy to forget he’s trans because of that.
They have done everything a normal couple has done but sex. This is why it is causing them some confusion. Society puts so much emphasis on dating, love, and sex. So much so that love and sex sometimes get tangled and become synonymous. But love comes in so many forms.
“Normal” couple? Plenty of romantic, ie “normal,” couples don’t have sex. These two, however, are queerplatonic, which thus means at least mostly non-romantic. So basically, you don’t have to love each-other romantically to be dating. THIS is what is causing them confusion…Well, I guess it could be both.

iirc in a previous comic, it’s hinted that Sarah will “take care of herself” if she wants, so I don’t think she’s particularly bothered by not having sex. She said something about how that part of her brain is totally separate from the part of her brain that likes cuddles and so on.
If anything, I would expect a manic adventure where Sarah buys a sexmachine and Noah tinkers with it – Sarah gets that particular physical need satisfied, while Noah gets the satisfaction of tinkering with a bit of technology and solving problems. And all the time, they’re arguing about what the machine should be doing, lol.

Satisfying yourself is not the same as sharing a physical connection with somebody, and a relationship can feel incomplete without that intimacy. (This is obviously not applicable to asexual folks.)

I love Noah and Sarah as a couple, and by now, they’ve gone from scene stealers to show stealers – but I’d also love to see Tara exerting her rights as older sister by showing Sarah a photo of pre-operative Noah (according to his bio, his full name’s “Noah Riley Ryan”… was “Riley” his birth name?)
That would be a massive dick move by Tara. I’m trans myself and a family member digging up a picture pre-transition just to serve as ammunition in some rivalry would be so far beyond the pale. Not to mention suggesting that Tara didn’t respect Noah’s identity because hey look, “he” had these tits and girly appearance, haha isn’t “he” silly right now?
It’s just… bad. Real, real bad and shitty thing to do. At least that’s how it would read to me.
I think they’re both being mature, just in different ways.
Noah is trying to think things out to figure out how things will be in the future, which is a mature thing to do.
Sarah’s already had time to think about it, because she had already accepted that it’s a ‘forever’ thing, so she’s trying to tell him not to overthink things… which is also a mature thing to do.
It’s just surprising to see Sarah being mature at all. At least, mature in a non-sexual way.
